New Patients Appointment Visits

New Patients must arrive 30 minutes before your appointment time.

Please bring the following items to your first visit:

  • Current Driver’s License

  • Completed Patient Forms

  • Current Health Insurance card(s)

  • Referral Forms, if required by insurance

  • A list of current medications, dosage and frequency

  • All previous diagnostic tests (X-Rays, MRI’s)

  • Information about your past medical and/or surgical history

Follow-Up Appointment Visits

Please arrive 10 minutes prior to your appointment time. Arriving more than 30 minutes late could result in your appointment being moved to a "work-in" or rescheduled as we must respect those who arrive promptly for their appointment.

Cancellations

Please notify us 24 hours prior to your appointment if you need to cancel or reschedule your appointment by contacting our office during normal business hours. Clearwater 727-734-6631 | Oldsmar 727-263-0800

No Show Appointments

If you do not show up for your scheduled appointment, you may be charged a $25.00 cancellation fee that will be your responsibility (this fee cannot be submitted to your insurance company).

Payment Information

It is our policy that payment is due at the time of service. Our office makes every attempt to verify your insurance benefits so that you will know what portion of your visit your insurance company is responsible for and what portion you will be responsible for, but we ask that you know and understand your own benefits to help ease the process.

We accept most insurances and will verify benefits prior to your appointment. Be aware that some insurance companies require a co-payment and a referral at the time of your appointment. We recommend you contact your employer or individual insurance provider for any questions regarding insurance or benefit coverage.
