Established patients are recommended to inform us if there are any changes to their health insurance plan or have had a lapse in insurance coverage. Failure to inform us will result in delay or denial in insurance payment. All copayments are due on the date of service. Deductibles and co insurance payments that are not covered by insurance will be collected on the date of service. Patients are recommended to contact the office ahead of the visit if there are any specific questions regarding referrals or insurance coverage.
